I understand that this class may be canceled or combined with another class if not enough students register but if I let other people know about the class it's less likely to be canceled.

ALL classes are Drop-off/Pick-up only. Family or Friends will not be able to sit in class and watch except if 3 year olds struggle at first. All students must follow dress code (no midriffs showing, no short-shorts or leotards without tights underneath, and no low necklines).

I give permission for my/our child to participate in Cedar Hills Dance. I give my permission for Cedar Hills Dance to use photos of my child for promotional purposes. I agree that I will in no way hold Cedar Hills Dance or its institutions/employees, responsible for any harm, injury, sickness, or death from participation in these classes. I also understand that my insurance will be the prime carrier in case of injury, accident, or death resulting from participation.
